• Assistance with personal care and ambulation as needed

Young at Heart Dementia Program  

• Registered Nurses on site
• Medical coordination with physician
• Assistance with personal care as needed
• Assistance to participate in activities
• Cueing and coaching to make the most of each day
• Structured programming to meet memory support needs:
  —Smaller groups – more assistance
  —Specialized activities to meet specific needs
  —Encouraging staff helps each participant realize success

• Transportation to and from the center
